He's talking to his love in the future. She's old and can't walk very well. He's also old. He's asking if their love will still be the same, and if she'll look the same. No she won't. You won't either dude.

And, baby, my heart could still fall as hard at 23
And I'm thinking 'bout how people fall in love in mysterious ways
Maybe just the touch of a hand
Well, me—I fall in love with you every single day
And I just wanna tell you I am
That's sweet, but what about after 70? Then you won't love her anymore?
This next line is adorable but grammatically unsound. I get what he means, but still.
There is truly a mystery about falling in love, cheesy as it sounds. He's speaking the truth.
I'm not sure that one touch of a hand can make you fall in love, but if it means a lot to you, think twice about the person whose hand you touched.
Falling in love daily, I've heard, is one of the keys to a long-lasting relationship/marriage. I don't know how to do it but I don't think it's something you know how to do; I think, rather, it's something that your heart propels you to do when the right person comes along.

This is the feel-good romantic part of love.
He's thinking out loud, he wants her to know his thoughts. This is one of the most endearing things a man can do: reveal his thoughts to the woman he loves.
Finding love where you are: what a concept these days! With the popularity of Match.com and going to great heights and depths to find love, did you think for just a second that maybe the love of your life is right under your nose, or someone you see everyday?

He has only a little hair left. I have a hard time with this because my preference is healthy hair that is not long but definitely there. His memory is also failing. What I have to remember is that my hair will probably be thinning and I'll probably forget more and more as time goes on. Such is this life.
He's not famous anymore, and that's okay. He's famous to me and I'll be there to remind him of that.
Those hands may not strum the strings the same, but don't fret honey (get it?) I still love you, and your playing will have a special quality that it didn't have before with your wavering, slower touch.

The concept of a young soul is an interesting one, and hearkens to the fact that our souls will never ever die.
He'll remember what she looked like when her eyes smiled from her cheeks, but don't count on that when she's old! Also, saying mind and memory is repetitive here.
When God sets forth a plan in your life, you are to follow it. If that plan includes falling in love with a certain person, do it. You won't have to expend hardly any effort if, again, it's with the right person.
Um...please don't keep making the same mistakes! Please grow and change and try to be better! I'll do the same! I won't understand you not trying to always be better. I would understand that as not caring enough about yourself or me to make any effort.
